Our ACH Authorisation Form Template streamlines the process of setting up electronic payments. This comprehensive form template is perfect for businesses, organisations, vendors and individuals to efficiently collect, authorise and process ACH payments.

An Automatic Clearing House (ACH) form grants financial institutions permission to transfer money from your bank account when a payment is due. Consumers submit ACH forms to businesses to consent to the authorisation process.

ACH authorisation is a type of electronic bank-to-bank transaction.

Employers, merchants, non-profits, utility companies and more use ACH forms to authorise a variety of transactions, including:

  • Authorising direct deposits, e.g. payroll, refunds
  • Setting up automatic bill payments
  • Collecting recurring donations
  • Enabling B2B payments
  • Initiating vendor/supplier payouts
  • Processing membership fees or subscriptions

The process of ACH authorisation involves:

  1. Agreement: The customer completes and signs an ACH form granting permission for a funds transfer.
  2. Verification: The business validates bank account details.
  3. Pre-Notification: Merchants send a test transaction to confirm account accuracy.
  4. Transaction Initiation: The financial institution processes payments or debits per the authorisation terms.
  5. Retention: The business keeps the authorisation on file for compliance.

ACH forms gather the details that are necessary to enable electronic payments directly from a bank account.

Our ACH form template has fields for personal details, bank account information, authorisation for recurring or one-time payments, and terms and conditions. You can distribute the ACH form via email or your website.

Customers may submit an ACH form to start a subscription or set up recurring rental payments. This form grants a business permission to charge them pre-approved amounts.

The business will verify the bank details, ensuring that all fields have been completed. A transaction is then initiated with the originating bank and sent to the receiving financial institute. Once processed, the funds are delivered to the business.

The terms and frequency will determine the transaction type. However, it is good practice for merchants to send reminders if the payment is recurring.

Customers submit an ACH authorisation to grant permission for direct payments, ensure secure transactions and simplify recurring payments.

Many businesses offer ACH transactions in addition to other payment options. For example, a leasing company may offer an ACH payment option for making rental payments. That said, no specific U.S. state mandates the use of the ACH form. It’s typically a business choice to comply with Nacha, the governing body of the ACH network and federal regulations.

Nacha rules and federal laws, such as the Electronic Funds Transfer Act (EFTA), govern ACH forms.

ACH authorisation forms capture details that are essential for processing electronic payments. Typical fields include the following:

  • The customer’s business name
  • The customer’s business address
  • The customer’s business address
  • The customer’s business email address
  • Customer information (name, email address, street address, phone number, etc.)
  • Payment information
  • ACH Terms
  • Authorisation information (routing number, account number, etc.)

You can customise the survey to include any questions that match your specific business needs.

  • What are the required fields to include on an ACH authorisation form?
  • Who should be in charge of managing ACH authorisation form submissions?
  • What can you use an ACH form for?
  • How should I manage incomplete ACH form submissions?
